The following are a few things that you, as a new patient, will need to do prior to or at the time you first receive care from our physicians:

• Understand the extent of your insurance coverage for office visits and any procedures that you are anticipating in the course of your care with our physicians.
• Complete our New Patient Forms
• Bring your driver’s license and your primary or secondary insurance cards.
• Bring with you a list of all current prescription medications and dosage information.
  

During your initial visit we will collect as much information as possible. However, when you return to our office, it is important that you always bring your current insurance cards. If you have a driver’s license, bring that too since it will help make the registration process easier for you. Also, it is important that you update us regarding any changes of address or phone number.
